What to attach
- Copy of invoice / violation notice
- E-ZPass account statement (if alleging tag was active)
- Vehicle registration
- Photos of tag placement (if obstruction is in dispute)
Pro tips
- If your tag was on the windshield but the camera read failed, attach your account history showing the tag in good standing on that date.
- For sold/transferred vehicles, attach the bill of sale and DMV plate-surrender receipt.
- Late penalties accrue fast — file the dispute even if you intend to pay the toll itself.
