What to attach
- Copy of HRA / agency notice (Notice of Decision)
- Case number
- Documentation supporting your position (income, household composition, medical, ID)
- Authorization for Representative form (if represented)
Pro tips
- File within 10 days of the notice to keep "aid continuing" — your benefits stay on while the appeal is decided.
- Cite the basis explicitly: "I disagree because [agency miscalculated income / failed to apply earned-income disregard / did not credit medical deduction]." Be specific.
- You can request the agency’s evidence packet before the hearing — do it.
